Log into your online account. Underneath 'System Links', click on 'Request CME Credit Processing'.

The Member ID request is the member ID or username used to log into the account.

Fill out the form being especially careful with the required fields. Missing any of these fields will cause the submittal request process to fail.

The first time a submittal is done for a course, please fill in all of the form including the feedback section. For future submittals in the same course, only the required fields are needed. You may fill out the feedback form or add any comments if you choose to do so. Your input is valued and always welcome.

Click the Submit My Feedback and Request button at the bottom of the page. You will immediately receive a confirmation page that your submittal has gone through.
